H. “Do Not Sell or Share” and “Limit Sensitive PI” links

If you use advertising/analytics that constitute “sharing” under California law, you should provide a clear mechanism on your website (commonly in the footer), such as:

  • Limit the Use of My Sensitive Personal Information

  • Or a combined “Your Privacy Choices” page/tool that supports these options

7) HIPAA, PHI, and Patient Information

Important: This website Privacy Policy is not a substitute for a HIPAA Notice of Privacy Practices (NPP).

  • When we are acting as a HIPAA covered entity, and we collect or maintain Protected Health Information (PHI) about you (for example, information in your medical record or designated record set), our use and disclosure of that PHI is governed by HIPAA and our Notice of Privacy Practices, as well as applicable state/federal laws.

  • Information you submit through the Services (for example, through appointment requests or messages) may become PHI if it is incorporated into your health record or used in connection with providing health care services.

  • You may request a copy of our Notice of Privacy Practices by contacting us using the information in Section 10, and it should be available on our website if applicable.


8) Data Security

We maintain reasonable administrative, technical, and physical safeguards designed to protect information. No system is completely secure, and we cannot guarantee absolute security.


9) Children’s Privacy

The Services are not directed to children under 13, and we do not knowingly collect personal information from children under 13.
